Steady As She Goes
A vessel grounded in a narrow channel under pilotage when the helmsman put the wheel over the wrong way. Both the Captain and Pilot realised that the vessel was swinging the wrong way and instructed the helmsman twice to increase helm, without realising that in each case, he increased helm the wrong way. Time elapsed 18 seconds at 18 knots, off track by 180 metres. The helmsman was experienced but very fatigued.
