Steady As She Goes

A vessel grounded in a narrow channel under pilotage when the helmsman put the wheel over the wrong way. Both the Captain and Pilot realised that the vessel was swinging the wrong way and instructed the helmsman twice to increase helm, without realising that in each case, he increased helm the wrong way. Time elapsed 18 seconds at 18 knots, off track by 180 metres. The helmsman was experienced but very fatigued.

Unconventional Use Of A Bucket

A Chief Stewardess was cleaning in the Owner’s accommodation and stood on upturned plastic bucket to wipe marks from deckhead panel. The bucket could not take her weight and collapsed, breaking the Stewardess’s ankle.

Lint Fire

A fire broke out in a laundry. On investigation, it was found that a large quantity of lint and rubbish had accumulated behind a drier and blocked the air intake.

Glass Does Not Bend

An ice cream tub was being used to freeze large quantities of ice for a party on board. A steward tried to put the whole lump in a glass water jug which would not fit, so attempted to force it to fit. The jug broke and the steward suffered severe tendon lacerations to his hand.

Cut Finger Leads To Fractured Skull

A Chef suffered a cut finger whilst preparing vegetables. His assistant left him to get help during which time he fainted, hitting his head against a worktop and fracturing his skull.

Theft Of Cash

Theft of cash in an Arctic port. Police attended on arrival with
customs and immigration officers etc. Captain received cash from
agents and was then distracted by police questions as he was
counting it. At some time between the first count and the final
count, five one-hundred dollar bills had disappeared.

Crushed Handling Boat

Crew recovering tender. Crew member stood between ship’s side
and tender to secure lifting hook, against company procedures.
Ship rolled and crewmember crushed, pronounced dead on scene.

Unreported Injury

Third Engineer took short cut down from generator during
overhaul and slid down casing to deck. Slipped and fell
injuring shin and face. Did not report accident. Ten days later,
leg had swollen. Medevac. Extensive tissue damage had become
gangrenous and required amputation.

A selection of precautionary tales experienced by the wider shipping community. If this serves to prevent one accident, it will be the most useful attribute of this website.

At the very least, once forewarned, we can learn from each other, take steps to protect the lives of our colleagues at sea and say, "There but for the grace of God, go I."
