Diving Safety

VLCC anchored in Singapore roads. Divers down replacing engine room intake grids. One diver experienced difficulty and was swept away and body recovered later. Divers report that surface currents in the anchorage are increasing in strength due to land reclamation. In this case, he was not wearing a securing lanyard which would have saved him.

Don’t Disturb The Captain II

Vessel not sending routine messages to Manager’s office during long sea passage. Various telephone calls answered by senior Officers but not Captain who was later found to have spent several days drinking heavily at sea.  Captain dismissed. Senior Officers disciplined for collusion.

Missing In Action

Deckhand drunk ashore missing for 24 hours. Police searched for him. ‘Shacked up’. Departure delayed due to US Immigration and Dept of Homeland Security intervention.

Hammer Time

Fitter using hammer. Hit own teeth.

Incorrect Tool

Tightening bolt with incorrect spanner. Slipped. Eye damage.

Hand Injuries

Deckhand greasing windlass. Inadvertently engaged gear. Glove and fingers went through gear wheel.

Securing For Heavy Weather

Chef taking garbage outside in heavy weather. Did not secure door which swung as the ship rolled, trapping his toes. 48 hours before he could be landed ashore. Necrosis had set in and his foot was amputated.

Vapour Burns

Messman tried to boil water in a glass jug in a microwave. When he opened the door, the water vapourised instantly and caused steam burns to face. Investigation explained that the rapid heating of water in a microwave oven does not allow time for ‘bubbling’ and that at the first sign of movement (possibly from the door being opened), this process starts instantly.

At the very least, once forewarned, we can learn from each other, take steps to protect the lives of our colleagues at sea and say, "There but for the grace of God, go I."
