US Navy Seal

Alongside in San Francisco, Duty Deckhand thought he had seen a diver. Reported to terminal control. Armed response. 72 navy divers attended. 4 vessels evacuated. Divers then claimed that visibility was less than 6 feet due to mud in suspension and decided the watchman had probably seen a seal.

Attacks ashore

Two incidents requiring hospitalisation ashore. Bosun returning alone at night in Buenos Aires. Mugged. Put up a fight and was stabbed repeatedly. Five seafarers ashore in Tallinn had taken drink and were flirting with local girls. Attacked by gang. Two men repatriated with severe head injuries.

Engine Room Slip

Third Engineer climbed onto generator to carry out routine maintenance. Slipped and broke jaw.

Tragic Practical Joke

Chinese crew. Practical joker pushed pressurised air hose against a Junior Seaman’s anus through his work clothes and turned air on. Seaman died of his injuries. Remorseful assailant later attempted suicide.

Fatality Outboard

Crew working outboard. A Deckhand slipped and was lost overboard. Suspected that inflatable lifejacket did not operate. Body not found.

Mobile Chef

Cook using cell phone and lifting hot pan simultaneously. Dropped pan. Burned feet (flip flops) – passed out and hit head on corner of range.

New Year’s Resolution

Chief Engineer suffered heart attack jogging. This officer was not a regular exerciser and had made a New Year’s resolution to lose weight and get fit. Medevac ashore in Russia.

Pressure of Work

Engineer opened up pressurised line and suffered facial injuries, Procedures in place but no permit to work issued.

At the very least, once forewarned, we can learn from each other, take steps to protect the lives of our colleagues at sea and say, "There but for the grace of God, go I."
